We are excited to launch the Fertility Friends Foundation (FFF) grant application!  These grants are intended to provide financial aid to individuals who cannot afford the fertility treatments they need. 

Day 1 Fertility Grant: If you'd like to be considered for a Day 1 Fertility Grant, note in the essay, why the support offered by the grant would be important for you.

The application form was created to make the process as easy as possible to complete.  It is imperative that you complete the application with all required information and attach all additional documents.  Incomplete applications will not be considered.

Step 1: Check out our check list that you will find in the downloads area below

Step 2: Include with your application

•      Piece of ID / Proof of citizenship / permanent residency

•      Copy of the front of your OHIP card

Step 3: Pay the mandatory application fees of $50 - The payment page will automatically appear once you submit your application form

Step 4: Send the medical evaluation's link, that you will find at the end of the application form, to your doctor.

IMPORTANT: on the Application Form, we are asking you to create a secret code composed of 6 numbers. This field is mandatory. You will need to communicate this code to your doctor as it must be included in your medical evaluation.

That way, your application form will automatically be matched to your medical evaluation once we receive it.

Step 5: IF YOUR APPLICATION IS PRE-SELECTED: Additional documents will be requested:

•       Credit Score

•       Copy of your T4

•       Signature of statement of no prior or pending conviction
